Apply for Housing
Individuals and families experiencing homelessness can apply for housing through the Coalition’s Housing Network. The form takes about five minutes.
What to Expect
1. You Apply
Fill out the short form below, or use one of the other ways to apply. You don’t need to include private details. We gather those later, in conversation.
2. We Reach Out
A member of the Coalition’s housing committee or outreach team will contact you, usually within 48 hours, to talk through your situation and next steps.
3. We Look for a Match
As housing becomes available, we submit applicants to network landlords. Landlords decide whether to accept a tenant, and tenants who are housed receive support through the Coalition’s Partners. Applying does not guarantee housing.
Who Can Apply
Individuals and families experiencing homelessness who have an income source, primarily Social Security or disability income, as well as those getting back into the workforce. Network housing has one additional requirement: tenants maintain sobriety, with no alcohol or substance use in any housing offered by network landlords. There are no fees to apply.
Other Ways to Apply
You don’t need internet access to apply. You can also email PutnamHomelessCoalition@gmail.com, call or text Red at 386-559-4023, speak with a Coalition member in the field, or ask another organization or advocate to refer you. If you’re applying through someone else, mention their name in the form or tell them to contact us.
Please don’t include Social Security numbers, bank or card numbers, ID numbers, or medical details in this form. We will talk through anything we need from you in person or by phone.

After You Submit

How We Use Your Information
Your answers are read by the Coalition’s housing committee and outreach team and used to find housing that fits your situation. With your consent on the form, we share the information needed for placement with network landlords and Coalition Partners. We do not sell or publish your information.
